Please be sure to bring a valid photo ID and insurance card. Please be advised you are still eligible to receive the vaccine, even if you do not have insurance or forget to bring it with you at the time of your appointment. If you are scheduling for your second or booster dose, please bring your vaccination card as well.

NOTE: We currently offer the Rapid & PCR Vaccines, but please note that the availability of vaccines are subject to change.
